Direct Billing
Please note that we need to be notified prior to your appointment if you would like direct billing.
Not all plans allow for direct billing, but we will do our best to find out if yours will.
OUR POLICY
If you would like us to direct bill, it is our policy to hold a credit card on file incase insurance companies to not pay in full.

You will also need to fill out and sign a consent form for us to direct bill upon arrival.
